Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)The hazards of performing leak testing (using pneumatic pressure) of tank trailer trucks. This testing was performed by employees without a written procedure, training, or knowledge of a consensus standard practice to follow. b)The hazards of pressurizing up a tank truck that was questionable in its ability to hold pressure. The employees introduced air pressure to 15 psig., when they did not know if the trailer was capable to hold that amount in that: 1.The tank had multiple leaks due to corrosion. 2.The age of the metal of the truck and lack of certification for the tank such as DOT certification or ASME certification would indicate that the tank may not be thick enough to hold the pressure. 3These leaks had been repaired by welds, no certified method of welding was used, and the welds and the tank was not inspected to determine if they were capable to withstand pressure. 4.A new gasket had been placed on the hatch cover. a)One method of abatement among others would be to develop a written procedure and train employees to pressure test tank trailer trucks based on a consensus standard such as IDOT 180.409 and IDOT 180.407 as guidelines. b)One method of abatement among others would be to have the tank trailers inspected and certified to meet the DOT criteria, have all repair welding performed by a certified welder, have the welds inspected for thickness and durability. **ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS ITEM.**

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)Employees at Ace Grease Service, Inc. were exposed to the hazard of being struck by falling loads, such as metal-fabricated grease bins, from an overhead electric hoist. The company did not have this electric hoist inspected on a frequent basis (daily to monthly) and periodic basis (at least every twelve months) to determine wear, deterioration and malfunction as per ASME B30.16-1998. One method of abatement, among others, would be to have this electric hoist inspected on a frequent basis, daily to monthly, and periodic basis (at least every twelve months) as required by ASME B30.16-1998. **ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS ITEM.**D
