Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a projectile metal roller on the coil winder: On or about February 5, 2003, at the end of the Pickle line at the coil winder, the employer failed to ensure that the metal control arm which secured the metal roller on the coil winder was securely held in place. The metal control arm above the roller broke while in operation and the roller went into the air, coming down and striking an employee, causing him to receive serious injury. The metal control arm had been previously broken and had been re-welded in place prior to this incident. Among other methods, one method of abatement, which has been accomplished since this accident, is to have repairs/welds approved and inspected by the manufacturer of the equipment and to install a safety strap to prevent the roller from releasing should the metal arm break. ***DOCUMENTATION OF ABATEMENT REQUIRED***ATION
