Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or about 6/7/05, employees were exposed to hazards associated with an excavator bucket becoming detached from an Esco Multi-Pin Grabber Coupler (i.e. quick coupler). Among other methods, a feasible and acceptable abatement method to correct this hazard would be to install the safety locking pin as described in the Esco Multi-Pin Grabber Coupler Installation and Operation Manual. **Abatement documentation required for this item**ed
