employment which were free from recognized hazards that were causing or
likely to cause death
or serious physical harm to employees in that:
Employees were supplying approximately 150 pounds per square inch of air
pressure to a
pumpless drain used oil system that had been repaired with a epoxy cement
type patch.  Failure of
the weld in this area caused the reservoir of the system to rocket into
the air and strike an
employee.
One feasible means of abatement, among others, is to inspect and evaluate
repairs of vessels
subject to air pressure from compressed air lines in the facility to
determine the effectiveness and
strength of these repairs.
