Employees were exposed to multiple injuries and death hazards created by
improper use of a
scissor lift that was not used in accordance with the American National
Safety Standard for Self
Propelled Work Platforms ANSI/SIA A 92.6-1990,sec 7.11.1: Personnel shall
maintain a firm
footing on the platform floor while working thereon.
(a) At the west side of the building, an employee was exposed to a fall
hazard of
approximately 23 feet during the process of welding a bracket when
standing on a 2x8
placed on a midrail of the scissor lift.
Among other methods, feasible and acceptable means to correct these hazard
are to use alternate
methods such as using a Boom-Supported Elevating Work Platform with
harness and lanyard
attached or raise the scissor lift up higher.
