Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling loads from an overhead bridge crane when the chain sling that was being used slipped out of the crane hook: A) ROTO DEPARTMENT #10; ON OR ABOUT NOVEMBER 14, 2002, THE TWO CHAIN SLINGS THAT WERE BEING USED ON THE COOKE CORPORATION 3 TON BRIDGE CRANE #6214A WERE IMPROPERLY PLACED IN THE HOOK AS REFERENCED BY THE AMERICAN NATIONAL STANDARDS INSTITUTE B30.10, HOOKS, SECTION 10-1.5 PARAGRAPHS C, D, AND E. ONE FEASIBLE METHOD OF ABATEMENT WOULD BE TO USE A DIFFERENT TYPE OF CHAIN SLING THAT WOULD CAUSE THE LOAD TO BE CENTERED IN THE BASE (BOWL-SADDLE) OF THE HOOK TO PREVENT THE CHAIN SLING FROM SLIPPING OUT UNDER LOAD.
