Title 40.1-51.1.(a), Code of Virginia: The employer did not furnish employment and place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: At the 686 bridge area, an employee was grinding a threaded rod with a nut on the side of the abrasive wheel, installed on the Stihl TS400 cut off saw, which was prohibited by the manufacturers instructions labeled on the side of the wheel, exposing employees to the hazard of being struck by flying pieces of wheel. " ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS, TO CORRECT THIS HAZARD IS TO USE ABRASIVE CUT OFF WHEELS ACCORDING TO THE MANUFACTURERS RECOMMENDATIONS"
