Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by the load should the overhead
hoist, supporting
structure or below the hook lifting device fail:
a)On or about 06/19/01: The following overhead hoists, had improperly rated
supporting structures, were not being inspected on a frequent and periodic
basis, support structures with unknown rated load capacity and missing or
broken safety latches, including but not limited to:
1)Hammondsport, Plant 1 Tool Room: Monorail rated for 1000 pounds
was equipped with a Yale 1-ton hoist, and four Budget 2-ton hoist
were not being inspected on a frequent and periodic basis.
2)Hammondsport, Plant 1 Inspection Area: C&M 1/2-ton hoist was not
being inspected on a frequent and periodic basis.
3)Hammondsport, Plant 1 Burring Room: Yale 1-ton hoist was missing
a safety clip on the hook and was not being inspected on a frequent
and periodic basis.
4)Hammondsport, Plant 1 Balcony Area: Loadstar 1/2-ton hoist was not
being inspected on a frequent and periodic basis.
5)Hammondsport, Plant 3 Wash/Tank Line: Two Sheppard Niles 1-ton
hoists were missing safety clips, were not being inspected on a
frequent and periodic basis, and the monorail had no rated load
capacity.
6)Hammondsport, Plant 3 Maintenance: C&M 2-ton hoist and C&M
1/2-ton hoist were not being inspected on a frequent and periodic
basis.
7)Hammondsport, Truck Maintenance: Loadstar 1/4-ton hoist was not
being inspected on a frequent and periodic basis.
8)Hammondsport, Plant 7, Welding Line 3: 3M 1/2-ton hoist was not
being inspected on a frequent and periodic basis and support structure
had no rated load capacity.
9)Hammondsport, Plant 7, Welding Inspection Bay 9: C&M Loadstar
2-ton hoist was not being inspected on a frequent and periodic basis
and support structure had no rated load capacity.
10)Hammondsport, Plant 8, Line 6 & 7: Loadstar hoists on gantry system
were not being inspected on a frequent and periodic basis.
11)Hammondsport, Plant 5 Garage: Loadstar 1-ton hoist was attached to
a 1/2-ton trolley. The hoist and trolley system was attached to the roof
trusses with no tests performed confirming the load rating of the
system (without an approved design indicating that it would withstand
the load and forces imposed by the load), and the safety clip was
broken on the hook.
12)Hammondsport, Plant 1 AEM Area: Loadstar 1/4-ton hoist was not
inspected on a frequent and periodic basis and the support structure
had no rated load capacity.
13)Mercury Central Maintenance: C&M 2-ton hoist was not inspected on
a frequent and periodic basis.
14)Mercury Central Tool Room: Two C&M 4-ton and four Budget 2-ton
hoists were not being inspected on a frequent and periodic basis and
the support structure had no rated load capacity.
15)Mercury Central Molding Area: C&M Loadstar 6-ton hoist on a
gantry system was not being inspected on a frequent and periodic
basis
Among other methods, one feasible method of abatement is to:
1) Inspect the support structures for the hoists as per ASME B30.16-1993,
and ensure hoist capacity does not exceed that of the support structure.
2) Conduct frequent and periodic inspections of the hoists for wear,
deterioration and malfunction as per ASME B30.16-1993.
3) Replace the missing and broken safety clips on the hooks.
b)On or about 06/19/01: The following below the hook lifting devices had no
rated load capacity markings, no initial, frequent and periodic inspections
performed, including but not limited to:
1)Hammondsport, Plant 1 Inspection: Below the hook lifting device had
no rated load capacity markings and no inspections were performed.
2)Hammondsport, Plant 1 Balcony Area: Below the hook device had no
rated load capacity markings and no inspections were performed.
3)Hammondsport, Plant 3 Wash/Tank Line: Below the hook lifting
device had no rated load capacity markings and no inspections were
performed.
4)Hammondsport, Plant 7 Weld Inspection Bay 9: Below the hook lifting
device had no rated load capacity marking and no inspections were
performed.
5)Hammondsport, Plant 7 Welding Line 3: Below the hook lifting
devices had no inspections performed.
6)Hammondsport, Plant 8 Line 6 & 7: Below the hook devices had no
rated load capacity markings and no inspections were performed.
Among other methods, one feasible method of abatement is to:
Conduct frequent and periodic inspections of the lifting devices and mark
the
lifting device with the rated load capacity as per ANSI B30.20-1993.
c)On or about 09/26/01: The following underhung crane was not being
inspected on a frequent and periodic basis, and did not operate safely.
Hammondsport, Plant 6 Multi Drill Area, Sheppard Niles 3-ton
underhung crane would not move south on crane rails, and was not
being inspected on a frequent and periodic basis.
Among other methods, one feasible method of abatement is to:
Conduct frequent and periodic inspections of the underhung crane as per
ANSI B30.11-1980.
Abatement documentation must be submitted for items a,b, and c.
