Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an amputation hazard: a)Fabrication Area, on or about 9/12/02: Hydra Tool Press Brake used to bend and form metal parts was not provided with safeguarding to protect operators who place their hands 1/2-inches from the point-of-operation. One feasible and acceptable abatement method to correct this hazard is to implement an effective "Safe Distance" Safeguarding Program as described in ANSI B11.3-1982, Safety Requirements for Construction, Care and Use of Power Press Brakes. Elements of the program include: a) Exposure prevention procedures; b) Training; c) Retraining; d) Supervision; e) Periodic inspections. Abatement certification must be submitted for this item.
