Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards: a)Press Area, on or about 1/15/2003: Presses, including but not limited to a Pacific Hydraulic Press Brake and a Sturdybender Mechanical Press Brake, used to bend and form metal parts were not provided with safeguarding to protect operators who place their hands approximately 2-inches from the point-of-operation. One Feasible and acceptable abatement method to correct this hazard is to implement an effective "Safe Distance" Safeguarding Program as described in ANSI B11.3-1982, Safety Requirements for Construction, Care and Use of Power Press Brakes. Elements of the program include: a) Exposure prevention procedures; b) Training; c) Retraining; d) Supervision; e) Periodic inspections.
