Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to rotating parts of rollers: a/ Middlesex Site/Production Extruder Area - 'Sano' Extruder, Extruder #2 and Extruder #6 Front End Rewind Areas: Employees are exposed to rotating parts of rollers during splicing (rewind/ winding setup) operations. Observed 4/7/03. One feasible and acceptable method to reduce this hazard is to ensure that whenever a cut-over operation occurs, two(2) operators shall be present. One(1) operator shall make the actual transfer while the second shall stand by the controls to stop the drives incase of an imminent accident, as stipulated in the American National Standard for Plastic Machinery - Plastic Film and Sheet Winding Equipment (ANSI B151.5 - 1982, Section 4.4.)
