Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were working in a self propelled extendible boom aerial lift and were not utilizing an appropriate fall arrest system. Galena Bridge jobsite - Employees working in extendable boom aerial lift approximately 15 feet above grade were exposed to fall hazards. Employees were not utilizing a fall arrest system. NO CERTIFICATION OR DOCUMENTATION REQUIRED FOR THIS ITEM Among feasible means of abatement would be to comply with the American National Standards Institute, ANSI A92.5 for Boom-Supported Elevating Work Platforms.
