Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to hazards associated with working in a permit entry confined space. a)South Elgin jobsite - A confined space permit was not completed prior to allowing an employee to enter a sewer manhole approximately 13 feet deep. b)South Elgin jobsite - Atmospheric testing was not conducted prior to allowing an employee enter a sewer manhole approximately 13 feet deep. c)South Elgin jobsite - Ventilation or other protective measures were not taken prior to allowing an employee to enter a sewer manhole approximately 13 feet deep. d)South Elgin jobsite - Live sewer line was not isolated while an employee worked in a sewer manhole approximately 13 feet deep. e)South Elgin jobsite - Emergency rescue equipment was not provided for employee working in a sewer manhole approximately 13 feet deep. Among feasible means of abatement would be to comply with the American National Standards Institute, ANSI Z117.1-1989 Standard for Safety Requirements for Confined Spaces.
