to:
Employees at Field Container Company, L.P., Marseilles, IL, were exposed
to
the hazards of installing paper
rolls onto the #2 Press Roll Stand when the paper roll fell three feet to
the floor upon attempted installation:
1)  Procedures were not developed for use when the paper roll won't line
up with the chuck.
Among other methods, one feasible and acceptable abatement method to
correct the hazard is:
Install tape lines on floor as a guide for tow motor operators
2)Maintenance procedures were not in place to discover gearbox cracks and
associated equipment
failures before they occurred.
Among other methods, one feasible and acceptable abatement method to
correct the hazard is:
Develop, document and perform preventive maintenance inspections according
to a
more rigorous schedule than is currently used.
A 3000-5000 lb paper roll fell three feet to the floor from #2 Press Roll
Stand
due to a failure of the
roll stand gearbox and associated equipment.
