SECTION 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or about January 22,2003, furnace operators were exposed to hazards of a special purpose personnel elevator that did not comply with ASME A17.1-1996 rules such as: 1. Rule 1502.6 - The elevator was not equipped with a door. 2. Rule 1502.7 - The elevator was not equipped with a car safety device that was capable of stopping the car and its load. 3. Rule 1502.10 - The elevator was driven by a prohibited belt-drive. Methods of correction for each hazard include but are not limited to: 1. Installing a full height gate or door. 2. Installing a car safety device. 3. A proper drive mechanism such as screw or rack and pinion may by installed.
