Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a rig site where H2s may be possible as an inhalation
hazard:
Employees were working at a rig site which had an (H2s May Be Present) sign
posted on the gate entering drilling field. The employer had not providing
information and training while working in a possible hazardous H2s
location. The
employer had not provided the following as required in American Petroleum
Institute (API) RP 54-92 dated 1992 section 4.4.1 and RP 55 Second Edition
Section 5,6,7,9 dated February 15,1995 on Section (6.7)
windsocks,streamers,flags or other suitable devices for indicating wind
direction.
Have a (5.7) safety review,(5.8) supplemental training,(6.1) Personal
Protective
equipment,(6.2) H2s monitoring systems,(6.3) portable detection
equipment,(6.4)
breathing equipment,(6.7) wind direction indicator,(7.4) plan information
for
training & (9.4) drills and safe work practices. This occurred on March 5,
2003
at rig site #9 in Eagle Pass, Texas.
Among other methods, one feasible and acceptable abatement method to
correct
the hazard is to provide information and training,personal protective
equipment,H2s detective monitoring equipment,wind direction indicators
such as
windsocks,steamers,flags or other suitable devices for indicating wind
direction.

were exposed to a possible fire/explosion hazard:
Employees were working at a drilling rig site and did not have the driller
trained
on Blowout Prevention especially when the Toolpusher would leave the rig
site.
The employer had not provided the following as required in the American
Petroleum Institute (API) RP 54-92 Section 5 Operations 5.1.2 employees
should
be familiar with the recommended practices, 5.1.3 employees should receive
instructions in correct work methods,(a) instruction of the driller or
crew chief,(b)
indoctrination of new employees,(c) instruction of crew members on work
procedures and safe practices,(d) regular meetings,(e) good housekeeping
practices,(f) use and maintenance of personal protective equipment,(g)
general
safety education and 5.1.9 Blowout prevention equipment shall be
installed. This
occurred on March 5, 2003 at rig #9 site.
Among other methods,one feasible and acceptable abatement to correct the
hazard
is to provide information and training on the use and operation of the
Blowout
Prevention Equipment.
