On or before 8/4/03, at 8868 SE Robwyn Street, employees were operating a
Toyota Model
FGC15 fork lift s/n 10538 that was not equipped with seat belts.
One feasible and acceptable means of abatement is the installation of a
seat belt assembly as
referenced by the American Society of Mechanical Engineers/American
National Standards
Institute, ASME/ANSI B56.1 and offered by Toyota as a retrofit item.
PLEASE NOTE:  CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS
VIOLATION.
