Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards:
On or about 03/18/04 at 550 Eagles Landing Parkway, Stockbridge, GA: The
following
fall hazards were observed:
a) An employee was exposed to fall hazards, while a passenger on a
personnel platform
being lifted by a Skytrak Model B8400 forklift. The employee was observed
using the guardrails
of the platform to climb onto an adjacent roof at a height of
approximately 18 feet above the
ground.
b) An employee was exposed to fall hazards, while performing work from a
personnel
platform, which was operated by a Skytrak Model B8400 forklift. The
employee
was not
wearing personal fall protection at a height of approximately 18 feet
above the ground.
Feasible and acceptable methods include but are not limited to the
following:
a) Employers should reference the ANSI B56.5 Rough Terrain Fork Lift
Trucks. In
accordance with this standard, personnel are to remain on the platform
floor. Use of railings,
planks, ladders, etc., on the platform for purpose of achieving additional
reach or height is
prohibited. When utilizing lift trucks in conjunction with steel erection,
the operator of the
forklift must place the platform on the roof surface, within the
controlled decking zone, in order
to safely discharge workers.
b)  Employers should reference ANSI B56.5 Employees should use a full body
harness
and lanyard anchored to the personnel platform.
