General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by flying debris and ammonia caused by a pressure vessel explosion: a. On or about 03/16/2004, in the ammonia compressor room: The compressor discharge accumulator was not equipped with a pressure relief device. Abatement note: Among others; one feasible abatement method to correct this hazard is to follow the ANSI Standard for mechanical refrigeration B9.1- 1971 section 10.1.2. which requires that all pressure vessels be equipped with pressure relief devices. Abatement certification must be submitted for this item.
