were causing or likely to cause death or serious physical harm to

were exposed to being struck by flying debris and ammonia caused by a
pressure vessel
explosion:
a. On or about 03/16/2004, in the ammonia compressor room: The
compressor discharge accumulator was not equipped with a pressure relief
device.
Abatement note: Among others; one feasible abatement method
to correct this hazard is to follow the ANSI Standard for
mechanical refrigeration B9.1- 1971 section 10.1.2. which
requires that all pressure vessels be equipped with pressure
relief devices.
Abatement certification must be submitted for this item.
