employer did not
furnish employment and a place of employment which were free from
recognized hazards that

were exposed to skin lacerations and pulmonary embolisms from exposure to
high pressure
compressed air:
In the following instances compressed air used for cleaning was not
reduced to 30 psi or
less:
a)Barn 55 - The air gun was not equipped with a safety nozzle.
b)Hallway Between Barn 57 and 59 - The air gun was not equipped with a
safety
nozzle.
c)Barn 24 - The safety nozzle on the air gun was covered with tape.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to retrofit all air hoses with safety nozzles that restrict air
pressure to 30 psi
or less when dead ended.re

In the following instances the drivers of manure trucks were not utilizing
an operator
restraint system:
a)Truck 315 - The operator was not wearing a seatbelt.
b)Truck 319 - The truck was not equipped with a seatbelt.
c)Truck 320 - The operator was not wearing a seatbelt
d)Truck 463 - The operator was not wearing a seatbelt.
e)Truck 340 - The operator was not wearing a seatbelt.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to require truck operators to wear seatbelts.

a)Hall Between Barn 84 and 85 - The outlet did not have adequate tension.
b)Boiler House, Brooder Barns 3 and 4 - The unused opening in the transfer
switch
electrical box was not covered or enclosed.
c)Boiler House, Brooder Barns 3 and 4 - The outlet did not have adequate
tension.
d)Boiler House, Brooder Barns 3 and 4 - The junction box was not provided
with
a cover.
e)Boiler House, Brooder Barns 3 and 4, West Wall - The 14 gauge wire,
supplying
power to the 1L circulator motor, was equipped with a 20 amp fuse.
f)Boiler House, Brooder Barns 3 and 4, East Wall - The 14 gauge wire,
supplying
power to the 2L circulator motor, was equipped with a 20 amp fuse.
g)Boiler House, Brooder Barns 3 and 4, East Wall - The 14 gauge wire,
supplying
power to the 2R circulator motor, was equipped with a 20 amp fuse.
h)Boiler House, Brooder Barns 3 and 4, East Wall - The 14 gauge wire,
supplying
power to the 1R circulator motor, was equipped with a 20 amp fuse.
i)Brooder Barn 3, Main Hallway - The junction box, supplying power to the
Poultry Trainer, was not grounded.
j)Brooder Barn 3, First Floor - The electrical outlet, located in the
middle
of the
barn, was not grounded.
k)Brooder Barn 3, Second Floor - The electrical junction box was not
grounded.
l)Brooder Barn 3, Third Floor - The electrical junction box was not
grounded.
m)Brooder Barn 4, First Floor - The outlet did not have adequate tension.
n)Brooder Barn 4, First Floor - The 18 breakers in the electrical panel
were not
labeled.
o)Boiler House, Brooder Barns 1 and 2 - The 121 volt Tork timer was not
provided
with a cover or insert.
p)Boiler House, Brooder Barns 1 and 2 - The 20 amp outlet was not grounded.
q)Brooder Barn 1, First Floor - The Chore Time Poultry Trainer was not
grounded.
r)Brooder Barn 2, First Floor - The Chore Time Poultry Trainer was not
grounded.
s)Brooder Barn 6, Generator Room - The 120 volt Tork timer was not provided
with a cover or insert.
t)Tool Room, 20s Complex - The outlet did not have adequate tension.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is for all electrical equipment to be installed in accordance with
the provisions of
the National Electric Code.

trucks:
In the following instances defective manure trucks were operated:
a)Truck 315 - The horn was not operational.  The driver side floorboard
contained
a hole.  The inside of the truck was not maintained in a sanitary
condition.
b)Truck 319 - The front windshield was cracked.  The horn was not
operational.
The seatbelt was cut or removed.  The brake lights were not operational.
The
inside of the vehicle was not maintained in a sanitary condition.
c)Truck 320 - The passenger door would not latch and was held shut with a
bungee
cord.  The driver side brake light and rear directional were not
operational.  The
fire extinguisher was not mounted.  The passenger side window wiper was
worn.
The rear differential was leaking.  The inside of the truck was not
maintained in
a clean, orderly, and sanitary condition.
d)Truck 463 - The truck was not equipped with a fire extinguisher.  The
inside of
the truck was not maintained in a clean, orderly, and sanitary condition.
e)Truck 340 - The fire extinguisher was not mounted.  The inside of the
truck was
not maintained in a clean, orderly, and sanitary condition.
Among other methods, one feasible and acceptable abatement method to
correct
this
hazard is to repair defective manure trucks.
