Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls: a)Back Hallway and Kitchen Bakery: Employees used a step ladder as an extension ladder which was also an inappropriate height for the area in which they were going to access and they also exceeded the load rating of the ladder, on or about 6/12/03. Feasible and acceptable methods for abating this hazard are: 1)Follow the manufacturer's recommendations for use of the ladder, 2)Follow ANSI A14.5-2000 - Selection, Care and Use, Sections 9.1.3, 9.2.1.2, 9.3.1.2, and 9.3.2. 3)Train employees on the proper use of step ladders as per the manufacturers instructions. NOTE:ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM. Attached form or equivalent must be submitted. See page 2,"Notification of Corrective Action" for instructions.
