Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: On or about January 28, 2004, employees were exposed to a struck-by hazard from falling loads while using overhead hoists. Two hoists had wire rope sections with broken wires that had been identified for repair. The repairs had not been made and the hoists were routinely utilized. The hoists are used to position wire coils weighing 4,000 to 6,000 lbs. a)In the Rod Mill area, for the four ton, Rod Mill hoist #4 (Rod Mill Let-Off South). b)In the PMP #17 and #18 area, for the three ton, Yale hoist #31.
