Section 5 (a) (1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the recognized hazards of being struck-by
falling equipment
associated with lifting and carrying equipment that exceeds the capacity
of an underhung
hoist:
The employer does not ensure that the rated load of an underhung hoist is
plainly marked on
each side of the hoist. This violation was observed on or about October
27, 2009, in the
clean and coat house, where employees are exposed to the hazard of being
struck-by falling
debris resulting from lifting/carrying loads that exceeds the capacity of
the
under-hanging
hoist.
Among other methods, one feasible and acceptable means of abatement would
be to ensure
the appropriate maximum rated load is plainly marked on each side of the
underhung hoist.
Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the date of
this citation, the
employer must submit documentation describing the steps that is taking to
ensure maximum
rated loads are plainly marked on hoisting equipment.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which was free from
recognized hazards of the
escape of combustible wire drawing soap dust from equipment into work
areas, increasing the
risk of fire, deflagration, and/or explosion which are causing or likely
to cause death or serious
physical harm to employees:
a.This violation was observed on or about February 17, 2010, in the area
of PMP machines
located west of the locker rooms, the employer did not prevent the escape
of dust from the
drawing die areas on the wire drawing machine PMP number 29.
b.This violation was observed on or about February 17, 2010, in the area
of PMP machines
located west of the locker rooms, the employer did not prevent the escape
of dust from the
drawing die areas on the wire drawing machine PMP number 30.
c.This violation was observed on or about February 17, 2010, in the area
of PMP machines
located west of the locker rooms, the employer did not prevent the escape
of dust from the
drawing die areas on the wire drawing machine PMP number 31.
One feasible abatement method, among others, to correct the hazard
identified in instances (a)
through (c) above, is to implement the requirement of NFPA 654 (2006)
Sections 8.1.1 control
of fugitive dust through continuous suction to minimize the escape of dust
where
combustible
dust is liberated during normal operations.
Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the date of
this citation, the
employer must submit documentation showing that it is in compliance with
the standard,
including describing the actions it is taking to establish, communicate,
and implement written
procedures to prevent the escape of combustible dust from equipment.
