Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm in that a powered industrial truck was left unattended while an employee was in a raised work platform. a. Shark City job site. On or about April 26th, 2004 a powered industrial truck was not operated in accordance with the American National Standards for Low Lift and High Lift Trucks (ASME B56.1-2000). Deficiencies include, but may not be limited to, the following section: 4.17.3(n) A trained operator should be in position to control the truck or available to operate the controls. A feasible means of abatement would be to have an operator at the controls at all times when an employee is on a raised work platform. CERTIFICATION REQUIRED FOR THIS ITEM.
