Public Law 91-596 Section 5(a)(1) Each employer shall furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees while operating unsafe hydraulic and mechanical vehicle lifts. Maintenance Garage: Instance A: The right side swing arm lock for the Challenger vehicle lift in Bay No. 4 was broken. Instance B: Both swing arm locks for the Challenger vehicle lift in Bay No. 7 were broken. Instance C: Hydraulic safety lock in bay No. 3 was broken. Instance D: Safety pins for the hydraulic jack unit on the Hunter vehicle lift in Bay No. 6 were not releasing as designed. Instance E: The green light that indicates the release of the locking mechanism on Hunter vehicle lift in Bay No. 6 was not working. Instance F: Plastic bushings in the lift mechanism were broken on the Challenger vehicle lift in Bay No. 7. Instance G: Automotive Lift Institute ALI 4.4.4-1994 Employer did not follow the recommended inspection procedures of the lift manufacturer. Instance H: ALI 4.5-1994 Appropriate repairs were not made on damaged vehicle lifts before continued use was allowed. Instance I: ALI 5.1-1994 Periodic inspection procedures in accordance with the lift manufacturer were not established. Instance J: ALI 5.4-1994 A record of each periodic inspection was not prepared or maintained. Instance K: ALI 5.5-1994 Inspection frequency did not follow the recommendations of the lift manufacturer. Instance L: ALI 6.2.1-1994 When noted by the operator, the employer did not perform necessary maintenance procedures. Among feasible means of abatement the employer can comply with the Automotive Lift Institute Guidelines ALI ALOIM - 1994M
