Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to molten metal splashes: (a) Plant, Unit #1, West Furnace: On or about September 2, 2003, the company did not have any written ram line installation nor burn-in/sintering procedures for employees to follow when working with this furnace. As a result, on 9/2/03 burn in/sintering time frames, as specified by the furnace and refractory manufacturers, were not adhered to resulting in molten metal burning through the refractory lining and contacting the water coil resulting in a molten metal explosion. Among other methods, one feasible and acceptable means to correct this hazard is to follow both the furnace manufacturer and refractory manufacturer's recommendations on the proper installation procedures for new ram lining and proper burn-in/sintering a new ram lining,/sintering
