Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by hazard: a) Parking Garage South Ramp - Two employees were injured when a concrete tip hose violently whipped around caused by trapped concrete mix and/or trapped air. The employer did not provide a spotter for pouring operations to communicate to the operator changes in pump gauge status. The operator was operating the pump via remote control. Air was introduced into the pump due to his inattention of the pump attendant. This was in violation of the Concrete Pump Manufacturers Association Safety Standard for Concrete Pumps, Placing Booms and Delivery System section 27-3.1 and Operators Manual for Model KVM 43X page 38 on or about 3/2/04. NOTE: Abatement certification required.tion
