Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing hazards:
a) Kingsbury, Inc. - On June 13, 2004, an employee operating a Dynatrol
Bullard 46
inch Vertical Turret Lathe, Serial number 32293, was exposed to the hazard
of crushing
injuries after being struck by half of the bearing being bored, weighing
approximately
2000 pounds. Written safe work procedures were not developed and/or
implemented for
machining parts in accordance with ANSI B11.6-2001, Clause 4.2, regarding
the user's
responsibility for overall workplace safety of personnel.
Among other methods, one feasible and acceptable method of abatement for
this hazard
is to implement written safe work procedures for machining parts in
accordance
with
ANSI B11.6-2001, Clause 4.2, concerning "User's Responsibility", regarding
aspects
such as: identifying the recognized hazards for the tasks to be
implemented (See Clause
5.1, "Task Hazard Identification"); provide and use protective measures
required for
those tasks (See Clause 5.2, "Risk Assessment/Risk Reduction" - this risk
reduction
would include ensuring that the babbitt is thoroughly cleaned out of the
bolt holes on the
inside of the bearings, to facilitate joining them together; and using a
variation of the
brass "shims" between the bearing and the serrated jaws to reduce the
likelihood of stock
slippage); ensuring that tool holders or adapters are securely held by the
machine; and
instituting training (see Clause 9.6, "Training").
b) Kingsbury, Inc. - On June 13, 2004, an employee operating a Dynatrol
Bullard 46
inch Vertical Turret Lathe, Serial number 32293, was exposed to the hazard
of crushing
injuries after being struck by half of the bearing being bored, weighing
approximately
2000 pounds. The employer did not supply, maintain, and enforce the use of
work-
driving and holding equipment required to perform the operation specific
to the lathe.
Among other methods, one feasible and acceptable method of abatement for
this hazard
is to incorporate ANSI B11.6-1984, Clause 7.1.9, concerning "Work-Driving
and
Holding Equipment", regarding the responsibility of the employer to
supply, maintain,
and enforce the use of work-driving and holding equipment required to
perform the
operation specific to the lathe.
