The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle:     a) On or about February 7, 2018 the VTL Toshiba 110, Machine Center 23005 TUE 15 machine was defeated and/or damaged and did not prevent employee exposure to the point of operations and/or struck-by hazards.       b) On or about February 7, 2018 there was no safety interlock or equivalent device installed on the sliding door on the VTL Toshiba TUE 15 machine, serial number 440205, to ensure that the door was closed while the machine was in operation and to prevent the operator from being exposed to point of operation and/or struck-by hazards.       c) On or about February 7, 2018 the safety device was damaged on the Mori Seiki CNC Lathe, Machine Center 01011 and did not prevent the sliding door from being open during operation, exposing the operator to point of operation and struck-by hazards.        d) On or about February 7, 2018 the female portion of the safety interlock on the Mazatrol 640 Mazak CNC Lathe, was missing and did not prevent the sliding door from being open during operation exposing employee(s) to point of operations and struck-by hazards.        e) On or about February 7, 2018 the female portion of the safety interlock on the Mazak Mazatrol 640 CNC Lathe, Machine Center 1014, was missing and did not prevent the sliding door from being open during operation exposing employee(s) to point of operations and struck-by hazards.       f) On or about February 7, 2018 the safety interlock switch was not connected to the sliding door guard on the Springfield Vertical Grinder 42ERail, Machine Center 09004 and did not prevent the door from being open during operation exposing the operator to the point of operation and struck-by hazards.
