Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to:
Engine spaces, process deck, trawl deck:  On or about June 16, 2008 and at
times prior thereto,
the employer had not developed, documented, and utilized procedures for
the control of
potentially hazardous energy.  A sample of identified equipment where
multiple sources of
hazardous energy existed included main engines, refrigeration systems,
trawl gates, and live tank
doors.  Employees are exposed to caught-in, struck-by, and electrical
shock hazards when
working on the identified equipment.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
for the employer to follow the written "Lockout and Tagout Program"
developed by the
employer.
