employees were exposed to electrical hazards due to unsafe electrical
practices and
procedures when euthanizing boars in the genetic boar stud facilities.
(a)Seaboard Farms, Inc. & 53022 CR 12, Holyoke, CO 80734: Employees at
genetic boar stud Farms #26 and #27 were exposed to electrical hazards
through the use of unsafe work practices and procedures during the
electrical
euthanizing of boars. Employees were exposed to electrical shock, burns,
and
electrocution hazards due to conductive materials such as metal and wet
concrete around the euthanizing pens, uninsulated alligator clips attached
to the
boar, and an extension cord set attached to the clips which was then
manually
plugged into a 120-volt receptacle.
Among other methods, feasible and acceptable abatement methods to correct
this hazard is to
utilize ME Schermer Humane Stunners or bolt guns with the Schermer 21 cap
for bulls or
follow NFPA 70E, Standard for Electrical Safety Requirements for Employee

employees were exposed to electrical hazards due to improper installation
and
use of
electrical equipment:
(a)Seaboard Farms, Inc. & 53022 CR 12, Holyoke, CO 80734: Employees
were not protected from electrical hazards due to improper installation
and use
of electrical equipment. Employees were exposed to shock, burns, and
electrocution hazards, such as but not limited to the following.
1.Temporary relocatable power tap in Farm # 26 overloaded, and not
rated by a Nationally Recognized Testing Laboratory. The relocatable
power tap was rated for 15A, and was being overloaded with
approximately 17.6A. The power tap was being used in conjunction
with a temporary cord-set that was attached to the ceiling and used to
power the relocatable power tap and the center island. (UL 1363)
2. Two 16 gauge cord-sets (SJTW E1363065-F) were altered from the
manufacturers intended purpose of providing temporary power. The
cord set had the insulation cut back, exposing the inner conducters
which were spliced into alligator clips used for euthanzing boars at both
farms # 26 and # 27. The ground prong was also missing on the cord-
set being used at farm # 26.
3.Main panel at farm # 26 had two circuit breaker openings that were
covered with electrical tape. The electical circuits behind the openings
were energized.
4.A temporary relocatable power tap that was daisy chained into a
temporary power cord to a plug in, located in the back hallway of farm
# 27. The power tap was powering a sensor that was used to open air
dampers for the cooling cell system which was located permanently in
the back hallway.
5.Two instances of branch circuits not legibly marked in panels located
on farm # 277. Approximately 10 electrical panels that did not have the
branch circuits legibly marked at farm # 277.
6.A 1/2" gap located in an electrical panel in the tool room of farm # 161
7.Fixed wiring (romex cable assembly) pulled through an opening of a set
of wooden shelves located in the generator room of farm # 161. The
cable assembly was not protected by a raceway.
8.An outlet receptacle located on the wall adjacent to the woodenshelving
in the tool room of farm # 161 did not have a face plate, with
energized parts within the outlet device.
9.Junction box located in the power washer room of farm # 161 had the
conduit/raceway pulled back from the box, exposing inner conductors.
10.Loose cover located on an electrical box on the feed auger in farm #
163.
11.Strain relief issue involving power cords for the AP feed augers in
room # 9 of farrowing at farm # 23.
12.Knockout missing on control box to the AP feed auger in farm # 118
13.Two light fixtures pulled from the ceiling exposing conductors
approximately 7'8" off the floor in both storage rooms of farm # 25.
14.Refrigerator used to store semen had an exposed power cord that had
been spliced and covered with electrical tape to protect the splice at
farm # 25. A temporary power cord that was attached to the west wall
of the boot room was providing electrical from an outlet on the other
side of the room.
15.Light fixture pulled from the ceiling exposing the inner conductors
located in the west gestation barn of farm # 22.
16.120 Volt plug-in receptacle located below an electrical panel in a
hallway of the east gestation barn, was damaged with inner conductors
exposed through the conduit at farm # 22.
17.Two instances of exposed inner conductors on motor housing flex
conduit connections on feed auger at farm #22.
18.Exposed inner conductors on fuse boxes located in farm # 22. A outlet
receptacle missing it faceplate and exposing the inner conductors in the
break room of farm # 22.
19.Branch circuits not labeled on a breaker panel located in farm # 55.
20.Two instances of exposed inner conductors located on a fan motor in
farrowing of farm # 55.
21.Circuit breaker openings of 1/2" observed in electrical panel of farm #
56.
22.An electrical cord with it's insulative jacket compromised and the
innerconductors split and exposed, covered with electrical tape. The power
cord was used for the mister in farm # 116.
23.Flourescent light bulbs not protected under 8 feet in height at farm #
26.
24.Conduit raceway pulled from a structural member in farm # 444. The
conduit was sticking out approximately 3 inches from the post.

knocked over.  The carbon dioxide cylinders were being used in the farms to
fill secondary containers for needleless injection systems associated with
vaccinating the hogs.

did not ensure that each employee, who operated the 1998 Artic Cat All-
Terrain Vehicle (ATV) as a means of transportation to travel between Farms
#449, 276, and 277, was provided with ATV operator safety training, in that
each employee was exposed to collision and rollover hazards during routine
maneuvers such as turning and driving on hills, loose, sandy soil and over
obstacles.
Among other methods, feasible and acceptable abatement methods to correct
this hazard is to
follow manufacturer's operating instructions located in the ARTIC CAT 1998
Operator's
manual, Division I-Safety:
(pg.11)Potential Hazard
Operating this ATV without proper instruction.
What Can Happen
The risk of an accident is greatly increased if the operator does not
know how to operate the ATV properly in different situations and on
different types of terrain.
How to Avoid The Hazard
Beginning and inexperienced operators should complete the certified
training course offered. They should then regularly practice the skills
learned in the course and the operating techniques described in this
Opertor's Manual.
