AMENDED CITATION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that:
a)On or about 10/26/04, the pallet system along the west wall, Aisle A of
the Wet Produce Room was not repaired according to the manufacturer's
recommendations and/or industry standards, in that wooden shims were
used to repair the rack resulting in an employee injury when appoximately
100 feet of rack collapsed.
Some feasible means of abatement include, but are not limited to the
following:
1. Maintain, repair, and/or modify metal rack system to meet the
requirements of ANSI MH16.2-1984 or newer.  Section 1, paragraph 1.4
Base Plates and Anchoring and/or rack manufacturer's recommendations.
