Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to potential inhalation hazards of ammonia:
1.  In the machine room, self-closing doors were not installed to the
machine
room (ANSI/ASHRAE 15-2001, Section 8.12.a).
2.  Remote control of the mechanical equipment was not provided immediately
outside the machine room, nor was a control switch provided for the
ventilation in that room,  (ANSI/ASHRAE 15-2001, Section 8.12.i).
3.  Systems containing more than 110 lb (50kg) of refrigerant were not
provided with durable signs having letters not less than 0.5 inches
(12.7mm) in
height designating the kind of refrigerant contained in exposed piping
outside
the machinery room (ANSI/ASHRAE 15-2001, Section 11.2.2).
4.  The Machinery Room did not conform to Class 1, Division 2, of the
National Electric Code when the employer provided neither continuously run
mechanical ventilation systems that would actuate alarms upon failure of
the
ventilation system, nor detectors located in areas where refrigerant
(ammonia)
from leaks would concentrate, that would actuate alarms and mechanical
ventilation systems at concentrations of 1000 ppm ammonia (ANSI/ASHRAE
15-2001, Section 8.12).
5.  An alarm set at a value not greater than the OSHA PEL (50 ppm) and a
detector for ammonia leaks were not provided in the machine room
(ANSI/ASHRAE 15-2110, Section 8.11.2.1).
6.  Signs reading "Authorized Personnel Only" were not posted on at the
machine room (ANSI/ASHRAE 15-2001, Section 8.11.8).
7.  Legible permanent signs indicating the name and address of the
refrigerating system installer, number and amount of refrigerant, lubricant
identity and amount, and field test pressure applied were not provided in
the
machine room (ANSI/ASHRAE 15-2001, Section 11.2).
8.  Schematic drawings or panels giving directions for the operation and
flow
of the refrigeration system for emergency shutdown were not developed and
available (ANSI/ASHRAE 15-2001, Section 11.7.
9.  Emergency shutdown procedures were not developed or displayed in the
machine room (ANSI/ASHRAE 15-2001, Section 11.7).
Among other methods, one feasible and acceptable method to correct these
hazards are:
1.  Install self-closing doors to the Machinery room.
2.  Ensure the mechanical equipment in the machinery room can be controlled
via remote control. Ensure the control switches are located outside the
machine
room.
3.  Ensure that all piping is labeled with the contents of the piping.
4.  Ensure electrical devices are rated for Class 1 Division 2, or install
a
mechanical ventilation system that complies with ANSI/ASHRAE 15-2001.
Provide continuously run mechanical ventilation system that would actuate
alarms upon failure of the ventilation systems, or install detectors,
located in
areas where refrigerant from leaks would concentrate, that actuates alarms
and
mechanical ventilation at levels of 1000 ppm ammonia.
5.  Install "Authorized Entry Only" signs to the entrance of the machine
room.
6.  Ensure pipe flow diagrams and/or schematics are developed and
available,
showing the direction of the flow of ammonia in the complete refrigeration
system.
7.  Install permanent signs indicating the name and address of the
refrigerating
system installer, number and amount of refrigerant, lubricant identity and
amount, and field test pressure.
8.  Develop emergency shut down procedures and ensure they are permanently
placed in the machine room. Adhere to the safe design, installation, and
operation of ammonia refrigeration systems as prescribed by the
ANSI/ASHRAE 15-2001 (Safety Standard for Refrigeration Systems).

employees were exposed to potential inhalation hazards of ammonia:
(a) Roof Area - Pressure relief devices did not discharge to the
atmosphere at
a location not less than 15 ft (4.57 m) above the adjoining ground level.
The
pressure relief devices were located approximately 6 inches above ground
level
and six inches near a drain located on the roof (ANSI/ASHRAE 15-2001,
Section 9.7.8).
Feasible and acceptable methods to correct these hazards, among others,
include:
(1) Discharge ammonia from pressure relief devices to the atmosphere at a
location not less than 15 ft (4.57m) above the adjoining ground level, and
eliminate the possibility of vapors entering the drain located on the
roof.

1. Ensure that the operators manual is followed by ensuring the pallet
jacks are
maintained in operating condition.
