device attached to a CM Lodestar 1/2 ton overhead hoist had no rated
load capacity markings.
5.Heat Treat, on or about 11/17/04:  A below-the-hook lifting device
attached to a CM Lodestar 1/2 ton overhead hoist had no rated load
capacity markings.
Among other methods, one feasible and acceptable abatement method to
correct
these hazards is to:
1.Mark the rated load of the lifting device on the main structure or
equivalent in accordance with ASME B30.20-1993, Section 20-1.2.1.
b)The following overhead hoists had broken or missing hook safety latches:
1.Scrape Area for Bridgeport, Pole L97, on or about 11/9/04:  A Budgit
1/2 ton overhead hoist had a missing safety latch at the hook attaching
the hoist to the trolley on the jib.
2.Bridgeport Area, Pole K97, on or about 11/9/04:  A Budgit 1/2 ton
overhead hoist had a missing safety latch at the hook attaching the
hoist to the trolley on the jib.
3.Bridgeport Area, Pole J97, on or about 11/9/04:  A Budgit 1/2 ton
overhead hoist had a missing safety latch at the hook attaching the
hoist to the trolley on the jib.
4.Bridgeport Area, Pole H95, on or about 11/9/04:  A Budgit 1/2 ton
overhead hoist had a missing safety latch at the hook attaching the
hoist to the trolley on the jib.
5.Manual Mills, Pole K41, on or about 11/17/04:  A CM Lodestar 1/2
ton overhead hoist had a missing safety latch at the hook attaching the
hoist to the trolley on the bridge crane.
6.Matsuura Area, between Poles P35 and P37, on or about 11/17/04:  A
CM Lodestar 1/2 ton overhead hoist had a broken safety latch at the
lifting hook.
Among other methods, one feasible and acceptable abatement method to
correct
these hazards are to:
1.Install new safety latches on the hooks in accordance with ANSI
B30.16-1993 Section 16-1.2.9.
c)The following monorails, bridges and/or jibs had no rated load capacity
markings on them, the pendant controls were not kept legible and the
supporting structures for the hoists were underrated for what the hoist
capacity was rated for:
1.Scrape Area for Bridgeport, Pole L97, on or about 11/9/04:  A jib
crane with a Budgit 1/2 ton overhead hoist attached had no rated load
capacity markings on it.
2.Bridgeport Area, Pole K97, on or about 11/9/04:  A jib crane with a
Budgit 1/2 ton overhead hoist attached had no rated load capacity
markings on it.
3.Bridgeport Area, Pole J97, on or about 11/9/04:  A jib crane with a
Budgit 1/2 ton overhead hoist attached had no rated load capacity
markings on it.
4.Bridgeport Area, Pole H95, on or about 11/9/04:  A jib crane with a
Budgit 1/2 ton overhead hoist attached had no rated load capacity
markings on it.
5.Assembly Area, between Poles F73 and G73, on or about 11/10/04:
A bridge crane with a CM Lodestar 1/2 ton overhead hoist attached
had no rated load capacity markings on it.
6.Assembly Area, between Poles F75 and G75, on or about 11/10/04:
A bridge crane with a CM Lodestar 1/2 ton overhead hoist attached
had no rated load capacity markings on it.
7.Manual Mills, Pole K41, on or about 11/17/04:  A bridge crane with
a CM Lodestar 1/2 ton overhead hoist attached had no rated load
capacity markings on it.
8.Steel Room, Pole C24, on or about 11/17/04:  A 3 ton Shepard Niles
underhung crane pendant control function labels were not legible.
9.Work Holding Chuck Cell, Pole F3, at the Hardinge Conquest VMC
#2861, on or about 11/17/04:  A CM Lodestar 1/2 ton overhead hoist
was mounted on a bridge crane structure which was rated for 1/4 ton.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to:
1.Mark the rated load on the main structure in accordance with ANSI
B30.11-1980, Section 11-1.1.2.
2.Mark the function labels on the pendant controls and keep them
legible in accordance with ANSI B30.11-1980, Section 11-2.3.3.
3.Prior to use, ensure the supporting structure is designed to withstand
the loads and forces imposed by the hoist for the rated load in
accordance with ANSI B30.11-1980, Section 11-2.2.2.
Abatement certification must be submitted for item a-4, a-5.
