Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by the load should the overhead
hoist, below-the-
hook lifting devices or bridge crane system fail:
a)The following below-the-hook lifting devices had no rated load capacity
markings and no inspections were conducted on the devices:
1.Basement, on or about 9/7/05:  A below-the-hook lifting device
attached to a 3 ton CM Lodestar overhead hoist #18 had no rated load
capacity markings on the device and no inspections were conducted.
2.Basement, at #5 pulper, on or about 9/7/05:  A below-the-hook lifting
device attached to a 2 ton Coffing overhead hoist #45 had no rated
load capacity markings on the device and no inspections were
conducted.
3.Beloit Winder Room, on or about 9/8/05:  A spreader beam below-the-
hook lifting device attached to a 5 ton CM Meteor overhead hoist #30
had no rated load capacity markings on the device, there was damage
on the right side of the beam where the J hook attaches to the beam
in that the bracket was bent outward, and no inspections were
conducted on the device.
4.Paper Machine Room, on or about 9/14/05:  A spreader beam below-
the-hook lifting device attached to a 5 ton Kinetic overhead crane #32
at the winder section for paper machine #3 was damaged on the right
side of the beam where the J hook attaches to the beam in that the
bracket was bent outward and no inspections were conducted on the
device.
5.Paper Machine Room, on or about 9/14/05:  A 3 ton spreader beam
below-the-hook lifting device attached to a 5 ton Saturn overhead hoist
#31A at the paper machine #2 reel section had not been inspected.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to:
1.Have the rated load of the lifting device determined and mark it on
the main structure or equivalent with ASME B30.20-1993, Section 20-
1.2.1.
2.Conduct frequent and periodic inspections in accordance with ASME
B30.20-1993, Section 20-1.3 and make repairs accordingly.
b)The following overhead hoists had worn chain links and missing safety
latches
on the lifting hooks and no annual inspections were conducted.
1.Basement, on or about 9/7/05:  A 3 ton CM Lodestar overhead hoist
#18 had worn chain links and no annual inspection was conducted.
2.Paper Machine Room, on or about 9/14/05:  Two 5 ton CM Cyclone
hand chain hoists #29 at paper machine #2 were missing safety latches
at the lifting hooks and no annual inspection was conducted.
3.Paper Machine Room, on or about 9/14/05:  A Saturn 5 ton overhead
hoist #31A at the reel for paper machine #2 had a broken safety latch
at the lifting hook and the pendant control was not labeled as to its
functions and no annual inspection was conducted.
4.Core Cutting Area, on or about 9/14/05:  A CM Shopstar overhead
hoist #20 was not provided with an upper limit switch and no annual
inspection was conducted.
5.Outside at the Headgates, a CM Meteor 2 ton overhead hoist #27, the
wire rope was kinked in several areas and the hoist and trolley would
not move freely on the monorail and no annual inspection was
conducted.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to:
1.Replace the load chain in accordance with ANSI B30.16-1993 Section
16-2.5.
2.Install new safety latches on the lifting hooks in accordance with ANSI
B30.16-1993 Section 16-1.2.9.
3.Label the pendant control functions in accordance with ANSI B30.16-
1993 Section 16-2.3(d).
4.Adjust the upper limit switches in accordance with ANSI B30.16-1993
Section 16-2.3(d).
5.Inspect the hoists on a frequent and periodic basis and perform the
required maintenance in accordance with ASME B30.16-1993, Section
16-2.
c)The following bridge crane structures for the hoists were rated for a
lesser
maximum weight than the hoist capacity was rated for and/or the rated
capacity for the supporting structure was unknown or not clearly
marked:1.Paper Machine Room, on or about 9/7/05:  Two Ingersoll Rand air
hoists rated for 5 ton each were mounted on a Roeper bridge crane
#23 structure which was rated for 9 ton.  Also, the rated load capacity
markings were not clearly marked on the Roeper bridge crane.
2.Outside at the Headgates, on or about 9/14/05, a monorail with a CM
Meteor 2 ton overhead hoist #27 attached, had no rated load capacity
markings on the rail.
3.Outside at the Flume gates, on or about 9/14/05, a monorail with a
CM Meteor 3 ton overhead hoist #26 attached, had no rated load
capacity markings on the rail.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to:
1.Prior to installation and use, ensure the supporting structure is
designed to withstand the loads and forces imposed by the hoist for
the rated load in accordance with ANSI B30.11-1980, Section 11-2.2.2.
2.Mark the rated load capacities on the bridge in accordance with ANSI
B30.11-1980, Section 11-1.1.
Abatement documentation must be submitted for items a-1, a-2, a-3, a-4,
a-5, b-2,
b-5, c-2 and c-3.
