General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a struck by hazard:
a)The following below-the-hook lifting devices had damage to the brackets
at
the J hook and no inspections were conducted on the devices:
1.Paper Machine Room, Paper Machine #3, on or about 7/23/09:  A
spreader beam below-the-hook lifting device attached to a 5 ton Saturn
overhead crane #32 had damage to the left and right sides of the beam
where the J hook attaches to the beam in that the brackets were bent
and no periodic inspections were conducted on the device.
2.Paper Machine Room, Paper Machine #2, on or about 7/23/09:  A
spreader beam below-the-hook lifting device attached to a 5 ton Saturn
overhead crane #31A had the retaining pin plates welded at the lugs
where the J hooks attached.  No operational tests were conducted after
repairs were made and no periodic inspections were conducted on the
device.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to:
1.Conduct frequent and periodic inspections of the devices in accordance
with ASME B30.20-1993, Section 20-1.3 and make repairs
accordingly.
2. Conduct operational tests of the devices after repairs and
modifications have been made in accordance with ASME B30.20-1993,
Section 20-1.4 and Section 20-1.5.
b)The following overhead hoists had a missing safety latch on the lifting
hook
and the pendant controls were not labeled:
1.Paper Machine Room, Paper Machine #2, on or about 7/23/09:  A 5
ton CM Cyclone hand chain hoist #33 had a missing safety latch at the
lifting hook.
2.Paper Machine Room, Paper Machine #2, on or about 7/23/09:  The
pendant controls for a Saturn 5 ton overhead crane #31A were not
labeled as to their functions.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to:
1.Install a new safety latch on the lifting hook in accordance with ANSI
B30.16-1993, Section 16-1.2.9.
2.Label the pendant control functions in accordance with ANSI B30.16-
1993, Section 16-2.3(d).
THE CELLU TISSUE CORPORATION WAS PREVIOUSLY CITED FOR A
VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD
OR ITS EQUIVALENT STANDARD WHICH WAS CONTAINED IN OSHA
INSPECTION NUMBER 307693432, CITATION NUMBER 1, ITEM NUMBER 1,
ISSUED ON 2/23/06 WITH RESPECT TO A WORKPLACE LOCATED AT 4921
ROUTE 58 NORTH, GOUVERNEUR, NEW YORK WHICH BECOME A FINAL
ORDER ON 9/12/07.
Abatement documentation must be submitted for items a-1 and a-2.
