were exposed to the hazard of injuries in the event of a tip-over accident
while operating a
powered industrial truck on or about 09/10/04.
On or about 09/10/04 at 180 Trans Tech Drive, Athens, GA:
a) Employee was exposed to the hazard of crushing injuries. Employee was
operating a forklift,
retrieving metal product from a materials rack, without the use of the
provided (restraint system)
seat belt.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS":
1) Instruct each employee during initial training and safety meetings,
emphasis to the operators,
to use the operator's restraint system (seat belts) in accordance with The
National
Safety Council
lift truck course.
2. Conduct periodic monitoring inspections of forklift drivers and correct
deficiencies
immediately.
