29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tag out devices and the responsibility for them.   On or about October 2, 2014 at 180 Trans Tech drive, Athens, GA:  Employees were exposed to sudden energization or start-up of multi-energy sources machinery. During the programmatic review of the lockout/tag out machine specific procedures for the milling machines and CNC machines it was revealed that there were deficiencies in the lockout/tag out specific procedures.  Specific procedural steps for placement, removal and transfer of lockout devices or tag out devices and the responsibility for them.    a. SNK #1 - Employees are exposed to an amputation hazard, there are not procedures to lockout and tag out the machine when a fault is found. The machine is turned off and the feed is turned to zero to clear the fault.   b. SNK #2 - Employees are exposed to an amputation hazard, there are not procedures to lockout and tag out the machine when a fault is found. The machine is turned off and the feed is turned to zero to clear the fault.   c. SNK #3 - Employees are exposed to an amputation hazard, there are not procedures to lockout and tag out the machine when a fault is found. The machine is turned off and the feed is turned to zero to clear the fault.   d. Mazak 1400 #1- Employees are exposed to an amputation hazard, when employee go into the CNC machines. The machine is manually stopped, the feed rate is turned to zero and the employees walk into the doors to check the parts being turned.   e. Mazak 1400 #2- Employees are exposed to an amputation hazard, when employee go into the CNC machines. The machine is manually stopped, the feed rate is turned to zero and the employees walk into the doors to check the parts being turned.   f. Makino #1 - Employees are exposed to an amputation hazard, when employee go into the CNC machines. The machine is manually stopped, the feed rate is turned to zero and the employees walk into the doors to check the parts being turned.   g. SNK #5 - Employees are exposed to an amputation hazard, there are not procedures to lockout and tag out the machine when a fault is found. The machine is turned off and the feed is turned to zero to clear the fault.

On or about October 2, 2014 at 180 Trans Tech Drive, Athens, GA.: Where employees were exposed to machine guarding hazard(s) to include but not limited to:  a. White Puck machine, when the Plexiglas guard was removed employees were exposed to an amputation hazard.  b. Cincinnati  Malacron milling machine, when the guard was removed, flying aluminum chips are thrown into the aisles that could strike the employees walking through the area. The milling machine is not guarded exposing employees to a struck-by hazard.  c. SNK#1, the key on the positive-break safety interlock was removed, allowing the door of the machine to be opened during operation and the employees to enter the machine. The employees were exposed to a caught-by hazard they venture into the machine.  d. SNK#2, the key on the positive-break safety interlock was removed, allowing the door of the machine to be opened during operation and the employees to enter the machine. The employees were exposed to a caught-by hazard they venture into the machine.  e. Makino#1, the positive-break safety interlock does not work. When the doors are open the machine will continue to run, exposing the employee to a caught-by hazard.  f. SNK#5, the positive-break safety interlock does not work. When the doors are open the machine will continue to run, exposing the employee to a caught-by hazard.
