Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The

furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to struck by hazards:
a) Plant #2: The employer did not maintain periodic inspection reports and
records for
overhead hoists to provide the basis for a continuing evaluation of the
hoists, on or

inspections
which include observations during operation to detect foreign objects
which may be
transported into the hoisting mechanism, on or about November 2, 2005.
Abatement Note: Among other methods, one feasible and acceptable method to
abate this
hazard includes: Comply with ANSI/ASME B30-16-2003, Section 16-2.1.1., and
the
manufacturer's recommendations for inspecting and maintaining the hoisting
equipment.
Assign responsibility and train personnel to perform frequent inspections
(at least daily,
weekly, and monthly) and periodic inspections (at least quarterly,
semi-annually, annually, or
at other intervals as recommended) depending on equipment use, and to
document, certify
and maintain inspection records.
