were exposed to fall hazards due to the use of boom-supported elevating
work platforms with
several operating deficiencies:
(a)Jobsite, 21 Century Drive, Ambridge, PA:  On or before 4-18-05
employees used the
boom-supported aerial platform, a JLG Lift, to gain access to the roof.
The lift had several
deficiencies which included, but not limited to; no markings or warnings,
illegible controls,
broken gauges, no operation and maintenance manual.
Among other means, one feasible and acceptable abatement method to correct
this hazard is to
ensure that the lift has the proper markings and warnings, legible
controls
and maintained
according to the manufacturer's recommendations and ANSI A92.5.
