Packaging P.R. Inc., there
was a metal mesh placed on top of the floor, this mesh was not secured or
fastened into the
floor, making a tripping and fallin hazard for employees when they walked
on the mesh that
moved and or displaced.  Employees alleged that they tripped and falled
because
of this
condition.

En el piso de produccion de Crown Beverage Packaging P.R. Inc al lado de
la maquina Ironer
#27 habia colocado un enrejillado de metal colocado sobre el piso, este
enrejillado no estaba
asegurado o atornillado al piso lo que creaba un riesgo de tropezones y
caidas cuando los
empleados caminaban sobre el mismo que se movia y desplazaba.  Empleados
alegaron haberse
resbalado y caido debido a esta condicion.

Employees exposed to tripping and falling hazards when walking over the
wire mesh false floor
placed in between the machines of the Crown Beverage Packaging Puerto Rico
Inc. plant.  The
was unleveled mesh and varied in height.  In front of Ironer #12 the mesh
extended 21 inches
and then the next mesh had a drop of 6 and half inches.   The employees
performed mechanical,
maintenance and operating task on the machines.
Among other methods, one feasible and acceptable method to correct this
hazard is to place the
wire mesh floor at the same level on all the places where it is used.
Hazard:  Tripping and falling
Empleados expuestos a tropezones y caidas cuando caminaban sobre el piso
falso de rejilla que
estaba colocado entre las maquinas del piso de produccion de la fabrica de
Crown Beverage
Packaging Puerto Rido Inc.  La rejilla variaba en altura lo que ocacionaba
que el piso falso no
estubiese nivelado.  Frente a la Ironer #12 la rejilla se extendia 21
pulgadas y la rejilla contigua
estaba a 6 pulgadas y media por debajo.  Los empleados realizaban tareas
de mecanica,
mantenimiento y operacion en la maquinaria.
Entre otros metodos, uno factible y aceptable para corregir este riesgo es
colocar todo el piso
falso de rejilla a el mismo nivel en todos los lugares que se use.

hazard is to provide
a working platform that employees could climb and work safely.
Hazard:  Falling
Empleados expuestos a caidas cuando se trepaban y se paraban sobre
motores, bombas, tubos
y otras piezas de la maquina "Washer" #3 localizada en ell piso de
produccion de la fabrica de
Crown Beverage Packaging Puerto Rido Inc.  Los empleados tenian que
realizar varias tareas
tale como: mecanicas, mantenimiento y operacion del "Washer", algunas de
la piezas y partes
de la maquina con la que los empleados tenian que trabajar no se podia
llegar a ellas a menos
que los empleados se trepasen o parasen sobre alguna de la partes que no
estaban disenadas para
ser plataformas de trabajo.
Entre otros metodos, uno factible y aceptable para corregir este riesgo,
es proveer de una
plataforma de trabajo en la cual los empleados puedan trapar y trabajar
seguros.

The ladder had 12 feet with 10 inches of height with 13 rungs.  Employees
had to use the ladder
for accessing the cat walks to perform maintenance tasks on the machinery.
The ladder was
located in the Cups area of the Crown Beverage Packaging Puerto Rico Inc.
production floor.
Among other methods, one feasible and acceptable method is to move the
ladder away from the
conveyor.
Empleados espuestos a tropezones, resbalones y caidas cuando usaban una
escalera fija de metal
que daba acceso a los andenes.  La escalera tenia un "conveyor" adyacente
en el piso que tenia
16 pulgadas de ancho.  La escalera tenia 12 pies con 10 pulgadas de altura
con 13 escalones.
Empleados tenian que usar la escalera para poder accesar los andenes para
realizar
tareas de
mantenimiento en la maquinaria.  La escalera estaba localizada en el area
de Copas en el Piso
de Produccion de Crown Beverage Packaging Puerto Rico Inc.
Entre otros metodos, uno aceptable y factible es depegar la escalera del
"conveyor".

Among other methods, one feasible and acceptable is place some kind of
soft cover on the metal
edge.
