Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that they were
exposed to fall hazards:
a)  St. Cloud, Florida Collection Route - A temporary employee sustained
fatal head
injuries when he fell from a rear loader refuse collection truck, on or
about
October 28, 2004.  The employer did not ensure the employee that
documentation was
made of the training provided and subsequent supervision.
Among other feasible and acceptable methods to correct this hazard is for
the
company to ensure that all temporary contract laborers working on rear load
refuse collection trucks receive documented training.  This training
should include
the use of the operator manual safety precautions, the job hazard analysis
for this
position and section 14.1.3 of the drivers and helpers safety manual.
