causing or likely to cause death or serious physical harm to employees
exposed to the hazard of
falls from a suspended scaffold.
a) 250 North Washington Ave. Goethals Bridge SINY. Employees engaged in
bridge refurbishing from a suspended scaffold, were exposed to falls of
over 100'
to the river below when no PFAS was provided and employer relied on guard
rails for fall protection. CSHO observed on or about 08/04/06.
Feasible and useful abatement methods for reducing this hazard, as
recommended by the
Fall Protection Standard, include but are not limited to the use of
personal fall arrest
systems.
b) 250 North Washington Ave. Goethals Bridge SINY. Scaffold was not built
to the
qualified persons design and presented defects including but not limited
to missing
thimbles on the main suspension ropes. CSHO observed condition on or about
08/06/06.
Feasible and useful abatement methods for reducing this hazard, to install
all scaffold
components as designed by the qualified person including, thimbles in the
main ropes,
softeners and chain link fence decking overlap of at least 12".
