Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm in that
employees were exposed
to being struck by a part weighing more than 9800 lbs when it was
improperly placed on a
positioning table that was not rated for the part. One feasible and
acceptable measure to abate
this hazard is to insure that parts are loaded onto tables that are rated
for loads that exceed the
weight of the part. This would include making calculations of the parts
center of gravity and its
distance away from the table and reducing the 10,000 lb load capabilities
of the table based on
this distance. In addition, load charts for these tables, which include
the maximum load ratings
for the positioners based on these measurements, should be affixed to the
positioners.
A) LOCATION: FABRICATION DEPARTMENT
THE PRESTON EASTON POSITIONER WAS OVERLOAD, CAUSING THE TABLE AND
THE 9600+ LB PART THAT WAS LOADED ONTO THE POSITIONER, TO BREAK
AWAY FROM THE BASE OF THE DEVICE AND FALL TO THE FLOOR WHILE THE
MACHINE WAS IN OPERATION. THE TABLE WAS RATED AT 10,000 LBS WITH THE
CENTER OF GRAVITY LOCATED 12" AWAY FROM THE TABLE. THIS MAXIMUM
LOAD RATING DROPS AS THE CENTER OF GRAVITY MOVES FURTHER FROM THE
SURFACE OF THE TABLE. THE PART, WHICH WOULD HAVE WEIGHED
APPROXIMATELY 9800 LBS AFTER THE WELDING WAS DONE ON IT, WAS 44"
HIGH, PLACING ITS CENTER OF GRAVITY FURTHER THAN 12" AWAY FROM THE
TABLE, AND EXCEEDING THE MAXIMUM RATED LOAD OF THE POSITIONER.
THERE WAS NO LOAD CHART ON THE POSITIONER.
