Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to potential fire and explosion hazards because methods and
practices were not
utilized to properly design and locate air-material separators (dust
collectors) to prevent
explosions and prevent propagation of a fire/explosion into employee
occupied areas:
(a)Inside Building - The Torit dust collector used to capture combustible
dust from
the Wheelabrator seventy two (72) inch swing table blast machine and the
Goff
spinner hanger model 2460 blast machine was located indoors, thereby
exposing
employees to a fire/explosion hazard, on or about July 14, 2006.
(b)Inside Building - The Torit dust collector used to capture combustible
dust from
the Wheelabrator seventy two (72) inch swing table blast machine and the
Goff
spinner hanger model 2460 blast machine was not provided with a
deflagration
vent to minimize pressure developed in the event of a fire and/or
explosion within
the dust collector, on or about July 14, 2006.
(c)Inside Building - The Wheelabrator seventy two (72) inch swing table
blast
machine and the Goff spinner hanger model 2460 blast machine were connected
by ductwork to the Torit dust collector and not provided with isolation
devices,
such as backdraft dampers to prevent deflagrations (fire/explosions) from
traveling through the ductwork between the pieces of dust producing
equipment,
on or about July 14, 2006.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable abatement
method to correct these hazards is to comply with the National Fire
Protection
Association (NFPA) 654, "Standard for the Prevention of Fire and Dust
Explosions from
the Manufacturing, Processing, and Handling of Combustible Particulate
Solids" (2006)
as it applies to your operations and equipment but not limited to:
* Locate dust collectors outside in accordance with Section 7.13.1.1.1 or
otherwise
adhere to the exceptions located in 7.13.1.1.2.
*  Protect dust collectors in accordance with 7.13.1.3 or otherwise adhere
to the
exceptions as listed.
*Provide isolation devices in accordance with 7.13.1.4 and 7.13.1.5 or
otherwise
adhere to the exceptions as listed.
