29 CFR 1910.94(a)(3)(i)(a): All air inlets and access openings were not baffled or so arranged that by the combination of inward air flow and baffling the escape of abrasive or dust particles into an adjacent work area would be minimized: (a) Mack Line - "Nu-Soft Steel Shot" leaked from the access openings on the Universal Touch-up cabinet, the Volvo Mack Touch-up cabinet and the BCP blast machine, on or about April 10, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Aisles and walkways were not kept clear of steel shot or similar abrasive which created a slipping hazard: (a) Mack Line - "Nu-Soft Steel Shot" was on the floor creating a slipping hazard, on or about April 10, 2013. 29 CFR 1910.95(g)(6): The employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels at least annually after obtaining the baseline audiogram: (a) Mack Line - The employer did not obtain an annual audiogram for one employee exposed to continuous noise at a dose of 67.4% which exceeded the permissible daily noise exposure of 50% (85 dBA) during 421 minutes of personal sampling conducted on April 18, 2013. The employee's average noise exposure was approximately 87.1 dBA. Zero exposure was assumed for the unsampled 59 minutes of the employee's shift. The primary health effect of overexposure to noise is hearing loss. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
