were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the following conditions:
(a)On or about 5/4/05, employees were exposed to the hazards of inhalation,
chemical burns, and uncontrollable release of chemicals such as, but not
limited to, anhydrous ammonia used for the nitriding process. Employees
routinely worked on and around piping systems which did not have the
identification of the contents labeled. Among others, one feasible and
acceptable method to correct this hazard is to comply with Paragraph 8 of
the American National Standards Institute (ANSI), A 13.1-1996 or of an
equivalent recognized and generally accepted standard. ANSI A 13.1-1996
states: "Positive identification of a piping system content shall be by
lettered legend giving the name of the content in full or abbreviated form.
Arrows may be used to indicate the direction of flow. Where it is
desirable or necessary to give supplementary information such as hazard
or use of the piping system content, this may be done by additional legend
or by color applied to the entire piping system, or as colored bands."

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the following conditions:
(a)On or about 5/4/05 and continuing thereafter, where employees are
exposed to the hazard of chemical releases and/or oxygen deficient
atmospheres:  At and around the nitriding process, which uses nitrogen
and anhydrous ammonia, as well as other heat treating process(es)
involving argon. There were no detectors to activate an alarm. One
feasible and acceptable method, among others, to correct these hazards is
to comply with multiple precautions for oxygen-deficient areas in CGA
(Compressed Gas Association) P-14 Accident Prevention in Oxygen-
Rich and Oxygen-Deficient Atmospheres, 1992, with particular attention
to the entire part of Section 6, Detection of oxygen-rich or oxygen-
deficient atmospheres.
