without wearing a seat belt while moving wire from the wire shop.

METHOD TO CORRECT THIS HAZARD IS TO: Require operators to wear seat belts
when
operating Tucker trucks and forklifts.

wheels of Mi-
Jack Travelift cranes.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS TO:  Enforce the requirement for
employees to
stay 6 feet away from the wheels of Travelift cranes.
(b)Prestress Yard, Casting Beds - During the period November 14, 2006
through
December 18, 2006 Tucker T-630 concrete trucks were used to fill forms for
bridge
supports with concrete.  Employees were observed walking in front of and
beside the
Tucker trucks in close proximity to the wheels while the trucks were in
motion during
the concrete pouring operation.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS TO: Develop operational procedures to
require
employees needed for concrete pouring operation to be in locations where
they are not exposed
to being struck by Tucker T-630 concrete trucks.
(c)Bulb- Tee Casting Beds, Form Storage System - On or about November 11,
2006,
pins for latches designed to secure forms in place while in storage were
missing and
some latches were damaged.  Post that latches are attached to were not
designed to
prevent upward movement.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS TO:  Replace missing latch pins.  Replace
damage latches.  Secure post in place.  Develop an inspection schedule and
conduct inspections
of form storage components.
(d)East Side of Prestress Yard - On December 19, 2006, a crane operator
operating
a Mi-Jack Travelift crane lifted a concrete pile over the an employee who
was positioning
cribbing on the ground for the load to be placed on.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS TO:  Establish and enforce work rules that
prohibit employees to walk, stand or work under crane or hoist load.
