On or about February 12, 2006,  an employee was using an oxygen acetylene
torch to cut a
piece of scrap steel from a coiled steel roll, and was fatally injured
when it released, struck,
and crushed him.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to:
Review and follow the employer's JSA # SH05 titled "Cutting Long Scrap to
Charging
Size"
for cutting scrap steel.
