employment
and a place of employment which was free from recognized hazards that
caused or were
likely to cause death or serious physical harm to his employees:
a.American Synthetic Rubber Company failed to effectively train employees
who
operated, and employees who performed maintenance and/or assembled
components of, the JetStream, Model 3015DHP, high pressure water jet unit
resulting in serious injury to an employee on August 8, 2005.  The hazard
was
recognized in that the incorrect assembly of a rupture valve on similar
equipment had caused serious injury to an operator on November 18, 2004.
JetStream, the manufacturer of the equipment provided training in the
operation and maintenance of this equipment to all of their customers
which
would  have  abated the  conditions.
